Sample Petition for Order of Protection

A victim of domestic violence may seek an order of protection by going to the circuit clerk’s office to obtain the forms to file a petition and affidavit. The Administrative Office of the Courts does not have an official petition; however, the circuit clerk’s offices are required to have all necessary forms. Many domestic violence shelters and some county offices also have victim advocates who can assist you with filing your petition and affidavit. There is no charge to file a petition for an order of protection.

If you are in immediate danger, dial 9-1-1.
